Group Life Insurance
The City of La Porte provides a $15,000 life insurance policy on all
eligible
employees. The cost of the plan is fully paid by the city.

Employee Assistance Program (EAP)
The Employee Assistance Program (EAP) is available to eligible employees
to
provide professional, confidential assistance for any type of personal problem.
The service is provided by New Avenues.
●Employees and their
families are eligible for four (4) free counseling sessions
per year.

What is WellPorte?
WellPorte will offer limited services, such as nurse practitioner
office visits, the
dispensing of prescription drugs and laboratory tests. WellPorte will provide
these
services to employees and dependents who are part of the City of La
Porte's
health plan free of charge:
●Primary Care - Colds, flu, asthma, diabetes,
etc.
●Preventive Care - Wellness testing,
physicals, etc.
●Pharmaceuticals - Generic medications
dispensed onsite
●Fast Easy Access - More time with a nurse
practitioner, but
less time waiting.

Wednesday: Closed
Tuesday & Thursday: 9:00 a.m. - 6:00 p.m.
